To help, we have put together a list of items that we will need from you before you can get started with your rehab program. This will save you some time on your first visit with us. Prior to your first appointment, you will need the following items:

Prescription for therapy from your doctor dated within the last 30 days

Copy of your Insurance card(s) and Drivers License/ID card (front and back)
